I would like to share with you why health care planning, also known as advance care planning (ACP) is so personal to me. Before practicing in this field of medicine, I was in a different specialty where I took care of patients who were very sick and needed to come into the hospital for emergent or urgent care. I felt as though I was a doing a good job meeting the needs of the patients and families we served.
I was mistaken.
Our Process
IDENTIFY
We identify individuals that would benefit from comprehensive advance care planning.
ENGAGE
We facilitate conversations with individuals and family on treatment and disease specific education.
DOCUMENT
Iris generates state-specific Advance Directives and other personalized documents.
DISTRIBUTE
We widely distributes advance directives and other documentation to the individuals, loved ones, caregivers, clinicians and points of care to ensure goals and plans are known and executed.
FOLLOW
We provide ongoing follow up to keep documents up-to-date and provide support in managing those care goals. We also provide resources on Palliative Care and Hospice as appropriate.
